吊装作业SOPHoistingStandardOperatingProcedure吊装作业标准操作程序
目的
规范吊装作业,防范因麻痹大意、误操作、违章操作等引起安全事故,保护职工在生产工作过程中安全和健康。
适用范围
本程序适用于在大中华区西得乐工厂及代表西得乐的现场的吊装作业,所有参与吊装作业的人员必须遵守本程序。
程序负责人
职责
程序负责人:
建立、推行、监督本程序,发生变更时或者至少每年回顾一次;
吊装设备负责人:
负责职责范围内的吊装设备日常检查,及故障跟踪;
各部门经理/班组长:
监督本程序的执行。
详细程序
1.定义
1.1起重机械:
吊运或顶举重物或物料的搬运机械,例如桥式起重机,门式起重机,流动式起重机,旋臂起重机等。
1.2(特种)起重机械:
额定起重量大于或者等于0.5t的升降机;额定起重量大于或者等于1t,且提升高度大于或者等于2m的起重机和承重形式固定的电动葫芦等。
1.3吊装设备负责人:
由部门指定的专人,负责日常检查吊装设备、跟踪并促进吊装设备安全使用状况。
1.4吊索具:
由专业制造商设计制作,用来吊装货物的器具。
2.操作特种起重机械进行吊装作业的司机,必须持“起重机械特种操作证”上岗。
3.班前检查:
开班前,班组起重机负责人要仔细检查与起重机安全运转直接相关的重要零部件的完好状况,并记录检查情况(见附件1),发现问题必须及时汇报、解决,未解决不得投入使用,防止事故发生。
4.定期维护保养
公司与有资质的维护保养单位签订起重机保养合同,保证专业的维护保养。
5.吊装安全操作:
5.1所有参与吊装的人员,必须精神集中,不得与别人闲谈。
5.2所有货物在吊装前必须试吊,在确保无安全隐患的情况下再正常吊运。
5.3吊装时,地面(包括工作平台)操作人员应远离工件可能掉落的范围。
5.4下列情况下,司机必须发出警告信号:
1.即将启动起重机前。
2.靠近同层其它起重机时。
3.起吊或下降货物时。
4.货物在吊运中接近地面工作人员时。
5.起重机在吊运过程中设备发生故障时。
5.5使用按钮1位,慢速启动,慢速停车,具体操作为:
按钮按下一半深度。
5.6悬臂吊和天车密集的flowingline区域,司机每次移动起重机前,必须确认相互没有干扰、搭连。
5.7不准用限位器作为断电停车手段。
5.8吊装前,司机必须了解货物的重量,对可能超载的未知货物,必须核实重量后,才能吊装;不准采用过载保护装置来权衡载荷。
5.9捆绑、挂钩(吊具)必须严格按照标准规范执行,具体见“工件吊装标准方法”。
5.10严禁吊钩、货物从作业人员头顶上方通过或停留。
5.11由起吊位置到达吊运通道过程中,货物必须高出其越过地面上最高设备0.5-1米,当货物到达通道后,必须降下货物,使其以离地面0.5米的高度随车运行。
5.12吊钩或货物不得不通过作业人员所在的上空时,司机必须连续发出警铃信号,待作业人员安全躲开后,方可开车通过。
5.13货物运达指定位置后,必须立即、慢速下降货物至地面、平台、或者工装上,严禁任何人站在悬空的货物覆盖下方,也禁止将货物悬空,无人看管。
5.14货物需要定位或旋转方向,必须将货物降落到距离协助者站立的平面1米以下,并无晃动后,才允许靠近。
5.15禁止用吊钩吊人或乘坐在货物上随货物移动。
5.16禁止使用两台起重机吊装同一货物。
5.17司机要严格遵守“十不吊”规定:
1.指挥信号不明确(两人及多人协调作业)或违章指挥不吊。
A.两人协作吊运物体时,动作必须协调一致,司机必须听从捆绑/挂钩/拆钩者的指挥。
B.多人共同吊运物体时,司机为负总责人,必须和其他组员逐一确认后,才能操作起重机。
2.超载不吊。
3.工件或货物捆绑不牢不吊。
4.货物上面有人不吊。
5.安全装置不齐全或动作不灵敏、失效者不吊。
6.工件埋在地下、与地面建筑物或设备有挂钩不吊。
7.光线隐暗视线不清不吊。
注:
当协同作业时,司机必须能够清楚的看到其他协作人员工作状态,否则严禁吊装!
8.棱角物件无防切割措施不吊。
9.斜拉歪拽工件不吊。
10.六级以上强风不吊。
6.吊装工作完毕后的安全操作
6.1为避免交通风险,司机要将吊钩升至足够高位置,不影响叉车及人员通行,但严禁将吊钩提升到安全限位器的顶部,吊钩上不准吊挂吊具、货物等。
6.2司机将起重小车停放在主梁远离大车滑触线的一端;大车应开到固定停放位置。
6.3按下急停开关。
6.4当班起重机负责人在下班前应对起重机进行检查,将工作中发生的问题及检查情况记录,并交接给下一班起重机负责人。
7.索具安全
7.1每次使用索具前,使用者必须对索具进行外观检查,发现有缺陷必须立即停用,汇报至班组吊装设备负责人。
7.2索具由HSE&S部组织每月定期检查。
7.3必须在索具设计承重范围内使用,严禁超载。
7.4当吊装的货物有尖角、棱边时,必须采取护套、护角保护索具和货物。
7.5每次使用前必须检查吊装带和链条等索具完全舒展开,并进行试吊,确保均匀受力。
7.6索具严禁多层缠绕或重叠使用,严禁打结或用打结的方法来延长长度。
7.7索具使用完毕后,不允许将拆除的索具从高空向下抛掷,以防止变形、意外砸伤或者勾挂。
7.8严禁用其它材料替代索具的任何标准配件。
7.9金属索具出现裂纹和变形时,严禁采用焊接和加热弯曲的方法修理;必须严格按照制造商建议进行修理或者替换。
7.10索具使用完毕要放在指定架子上悬挂存放,不能放到地上,避免发霉,虫咬,生锈;也不能堆放,或承压堆放。
7.11存放地点要通风、干燥,避开热源(包括火源和电气焊)或日光及紫外线长期辐射。
7.12当在酸碱场合使用后,应立即用凉水或清洗剂将吊装带冲洗干净,晾干贮存,避免发霉;严禁用酸碱等化学品清洗。
7.13专门设计定做的索具只能用于指定的货物吊装,不允许吊装其它货物。
7.14卸扣的正确安装方式,需使力作用在卸扣本身的弯曲部分和横销上,使用时不允许超过120°。
(竖直使用吊装时,为100%载额;45度以内使用,应使用额载的75%;90度以内使用,应使用额载的50%)
7.15吊钩只能竖直方向受力,严禁尖载、背载、侧载、夹载。
7.16报废标准
当出现下列情况之一时,应立即停止使用,更换新的,报废旧索具;遇到临界报废的索具或无法确认使用安全时,均做报废处理。

1无规律分布损坏:
在6倍钢丝绳直径的长度范围内,可见断丝总数超过钢丝绳钢丝总数的5%
2钢丝绳局部可见断丝损坏:
有三根以上的断丝聚集在一起
3索眼表面出现集中断丝,或断丝集中在金属套管、插接处附近、插接连接绳股中
4钢丝绳严重磨损/腐蚀:
在任何位置实测钢丝绳直径,尺寸已不到原公称直径的90%
5因打结、扭曲、挤压造成钢丝绳畸变、压破、芯损坏或钢丝绳压扁超过原公称直径的20%
6钢丝绳热损坏导致强度下降
7可见的在插接处、金属套管处的挤压、磨损(如裂纹、变形、腐蚀)
8绳端固定连接的金属套管或插接部分滑出
吊带
1吊带外皮破损
2合成纤维出现软化或老化(发黄)、表面粗糙、合成纤维剥落、弹性变小、强度减弱
3承载接缝绽开、缝线磨断
4吊装带发霉变质、酸碱烧伤、热熔化或烧焦、表面多处疏松、腐蚀。
5吊带起毛以至于看不清带纹,确认经纱损伤。
6吊装带出现死结。
7吊装带异常变形。
卸扣,吊钩,横梁吊具
1锈蚀和磨损
2发生变形。
3有裂纹和裂痕
链条
1链环发生塑性变形,伸长达原长度的5%。
2链环直径磨损达原直径的10%。
3链环出现裂纹、弯曲、扭曲或表面损伤现象。
8.工装安全
工装的设计必须由专业人员设计,不得随意更改设计,更改的设计必须经过有资质机构验证;
9.监督检查
部门经理,班组长负责日常工作的监督检查;

1.Definition
1.1Cranemeansthemachinerywhichisusedtolifttheheavygoodsincludingoverheadcranes,gantrycranes,mobilecranes,jibcranes,collectivelyreferredtoaslifting.
1.2(Speical)cranemeansthehoistingmachineswithcapcitylargerthan0.5KG,orcranewithcapcitylargerthan1tonandliftingheighthigherthan2meters.
1.3Craneownerisnominatedbydepartmentmanagerfordailyinspection,trackingandpushingsafetyuseoneachhoistingmachine.
1.4Riggingmeansthetooldesignedbycertifiedmanufacturetohoistthegoods.
2.Driverswhooperatethespecialcraneforhoistingtasksmusthaveeffective“craneoperationlicense”.
3.Inspectionbeforeshift
Thecraneownermustcheckthecriticalcomponentswhicharedirectlyimpactonmachinesafetyatthebeginningofeachshift,andtaketherecord(appendix1).
Anyproblemmustbepromptlyreported,resolvedbeforereuseinordertopreventaccidents.
4.Regularmaintenance
Regluarmaintenancecontractwithcertifiedsupplierissignedtoensuretheprofessionalservice.
5.Safeoperatingrequirements
5.1Anypersonnelinvolvedinliftingmustconcentrate,nochatwithothers
5.2Dohoisttrialbeforeeachnormalhoisting.
5.3Operatorsontheground(includingtheworkingplatform)muststayawayfromthescopeofthegoodsmayfalldown.
5.4Thedrivermustsendanalarmsignalinfollowingcases:
1.Thecraneisabouttostart.
2.Beingclosetotheother(jib)cranesatthesamelevel.
3.Liftingorfallingcargo.
4.Beingclosetogroundstaffduringliftingtransportation.
5.Equipmentfailureduringlifting.
5.5Slowstart,slowstopbypressingthebuttonhalfofthedepth(thefirstgear).
5.6Thedrivermustconfirmnointerferenceandjoinbetweeneachjib(crane)inflowinglineareabeforestarting.
5.7Itisnotallowedtousethelimiterasameansofstopping.
5.8Thedrivermustknowtheweightofthecargobeforelifting;Itisprohibittotesttheweightwithoverloadprotection.
5.9Bindingandhangingmethodsmustcomplywiththestandardinstructions.
5.10Itisforbiddentomakethecargostayingorgoingthroughthetopofanypersonnel’shead.
5.11Thecargomustbelifted0.5-1meterhigherthanthemaximumheightoftheequipmentsonthegroundbeforereachingtheliftingchannelfromthestartpoint;thenthecargomustbeloweredtomakeittotheheightof0.5metersfromthegroundwhenreachingthechannel.
5.12Thedrivermustsendanalarmsignalcontinuouslyanddrivethroughafterthepersonnelmakingwayincasethehookorthecargohastopassoverheadofanyperson.
5.13Thecargomustbelandeddowntothegournd,platformorjigsslowlyandimmediately;Itforbidsanypersonstandingbelowthesuspendedcargoandkeepthesuspendedcargounattended.
5.14Itisallowedtoholdthecargosonlywhenthecargoislandeddownwithin1meterdistancefromtheplanewheretheoperatorstands.
5.15Anypersonisprohibitedtomovewiththehookorthehangingcargos.
5.16Itisprohibittoliftonecargotogetherwithtwocranescooperation.
5.17Thedriversmustobeythe“Tennon-liftingcases”strictly:
1.Noliftingifcommandsignalisnotclear(twoormorepersoncoopration)orillegalcommand
A.Thedrivermustobeythecommandfromthepersonwhobindscargo/hook/untiethehookincaseoftwo-worker’scooperation.
B.Incaseofmulti-worker’scooperation,thedriveistheleaderwhomustmaketheclearconfirmationwitheachcollaboratorbeforeanyoperationonthecrane.

6.Safeoperatingprocedureswhenliftingisfinished
6.1Toavoidtheriskoftraffic,thedrivermustrisethehooktoahighenoughpositionthatdoesnotaffectthepassageoftrucksandpersonnel.
6.2Thedriverneedtoparktheliftingtrolleyattheoppositeendofslippy-touchwiresonthemainbeamcartandparkthecranetotheappointedlocation.
6.3Presstheemergencystopswitch
6.4Craneequipmentownermustcheckeachcranebyendofshiftandhandovertheproblemstothecraneequipmentownerofnextshift.
7.Riggingsafety
7.1Anyriggingmustbevisualinspectedfordefectsbeforeeachusebytheuser.Stoptousedefectiveonesimmediatelyandreporttotheteamliftingequipmentleader.
7.2HSE&Sdepartmentexecutethemonthlyinspectiononallriggings.
7.3Riggingmustbeusedwithinthedesignedcapacity,overloadisprohibit.
7.4Protectioncomponentsmustbeusedwhenlifitingwithtestileslings.
7.5Therigginglikeliftingbeltsandchainsmustbecheckedfullystretchandaverageforceonslingsbyliftingtrialbeforeeachuse.
7.6Overlappingormulti-layerwindingisstrictlyprohibited.Knotorextendthelenthoftheriggingbyknotisprohibitedaswell.
7.7Don’tthrowtheriggingdownfromheightaftereachusetoavoidunexpecteddistortion,collisioninjuryandhitching.
7.8Anyreplacementofthestandardriggingaccessoriesbyothermaterialisnotallowed.
7.9Itmuststrictlyfollowthemanufacturer'sinstructionsforrepairorreplacement.Weldingorreparingbyheatingmethodisprohibitedwhenthemetalriggingcrackordeform.
